Occult groin hernias are rare and clinically difficult to diagnose because they are not routinely looked for. MRI is the most accurate preoperative diagnostic tool in suspected cases. A sound knowledge of groin anatomy and meticulous preperitoneal inspection of all possible rare groin hernias sites should be done to diagnose and treat all defects. We are presenting a case of rare prevascular hernia which was diagnosed per-operatively and treated successfully.
